Philadelphia Paperwork

By now you’ve all received the vast selection of packets required to fill out prior to our May trip to Philadelphia: Our nation’s birthplace.  This is an unbelievably enjoyable and equally educational trip.  We hope everyone can join us!

In your packets there is a lot of information provided and may seem daunting.  First and foremost, the Commitment Form, located on the last page of your “Trip Overview” packet, must be submitted by Thursday, February 12th.  That is also the date of our fifth grade play if it helps you remember.  A deposit check must accompany this form. 

 

Other slips requiring completion once your Commitment Form is sent in include:

  1. The Code of Conduct Form (both pages)
    •  This has been provided as it’s own two page packet.
  2. The Medical Forms 
    • All parents (including chaperones) must fill out the first three pages. 
    • If a child is attending on their own, parents are required to fill out page four.  This page pertains to all and any medication you wish administered to your child while on the trip, including over the counter, i.e. Tylenol.
    • Again, this has been provided as it’s own seperate packet.

In total three separate pieces of information necessitate completion as soon as possible.  The Commitment Form is most important at this time.  At a future date we will also be sending home a box lunch selection form for the bus ride dinner home. 

This trip is always a highlight here in fifth grade.  We can’t wait for this year’s experience!
